Boundary survey and legal description for each parcel of property subject to the application but owned by separate and distinct persons, corporations, partnerships, or other legal entities; and prepared by a surveyor registered by the State under a surveyor's seal and certified to the City. If applicable, provide the gross and net land area which may be provided on a separate sheet. |

Analysis: Complete analysis of the proposed amendment (sections a through f of the procedures manual). |

Statement of Appropriateness: A general description of the proposed amendment to the comprehensive development plan, explaining why the amendment is appropriate or necessary. |

Phasing plan: Estimated timetable and method of financing for each public facility improvement necessitated by the proposed comprehensive development plan amendment. |
